同一药品院内院外不同价，国办发文引导合理定价

\u003cp\u003e国务院办公厅14日发布的《关于健全药品价格形成机制的若干意见》（下称《意见》），明确加强药品价格监测预警，对同通用名药品中价格明显偏高的进行价格风险提示；推进药品价格协同治理，强化药品价格和反垄断执法，依法严厉惩处药品和原辅料生产经营领域以缺逼涨、垄断涨价、操纵市场价格等违法违规行为。\u003c/p\u003e \u003cp\u003e医疗机构、零售药房和网上药店等相关主体需在“药品价格发现”中承担主体责任。其中，医疗机构需采取综合措施切断医和药之间的利益链。\u003c/p\u003e \u003cp\u003e受访业界人士认为，接下来，在全渠道的药品价格治理中，有关部门将采取技术手段与制度约束并重的策略，并在实践中逐渐厘清院内外、线上线下的价差合理性边界。\u003c/p\u003e \u003cp\u003e\u003cstrong\u003e引导不同渠道合理定价\u003c/strong\u003e\u003c/p\u003e \u003cp\u003e当前，同一药品在公立医院、实体药店和网上药店等不同渠道的价格差异问题受到广泛关注。\u003c/p\u003e \u003cp\u003e这背后存在一定的合理性因素。国家医保局医药价格和招标采购司司长王小宁15日在《意见》解读发布会上表示，目前公立医院是药品销售的主要渠道，实体药店是院外市场重要领域，近年来网上药店发展也是非常迅猛的。这三种渠道面临的市场交易条件不一样，价格形成逻辑也不同，所以表现在同样一盒药的终端价格也就可能存在一定的差别。\u003c!--MID_AD_0--\u003e\u003c!--EOP_0--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_0--\u003e \u003cp\u003e“但是有一个前提，就是不同渠道之间的价格差距不能太大，要在合理的范围之内。这是我们开展价格治理的重要着力点。目前，受竞争压力和治理规则的约束，不同渠道的价格正在逐步趋同、更加合理。”王小宁表示。\u003c/p\u003e \u003cp\u003e为此，《意见》提出要发挥医疗机构专业作用、引导药店合理制定药品零售价格、用好网上药店价格发现功能等措施，鼓励各类主体参与药品价格的形成。\u003c/p\u003e \u003cp\u003e“今后，全渠道价格联动会三管齐下。”上海市卫生和健康发展研究中心主任金春林对第一财经表示，根据《意见》，一是对医疗机构加强零差率药品销售的健全，医院所有药品均需通过省级平台采购，集采中选和协议期内谈判药不得议价，同时采取红黄标价格预警机制，向医疗机构提示价格风险，引导药品价格运行在合理区间；二是运用大数据、\u003c!--VERTICAL_CARD_BEGIN_0--\u003e人工智能\u003c!--VERTICAL_CARD_END_0--\u003e等新技术手段，探索对医保定点药店药品价格管理的有效做法，通过“公开比价+”的方式，依托医保信息平台大数据编制量价比较指数，引导医保药品价格更加合理；三是将比价和监测的机制延伸至网上药店，引导网络平台经营者加强平台内网上药店管理。\u003c!--MID_AD_1--\u003e\u003c!--EOP_1--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_1--\u003e \u003cp\u003e为促进不同渠道药品公平合理定价，尤其是促进网上药店与零售药房的医保药品实现公平定价，武汉大学法学院副教授、大健康法治研究中心副主任周围进一步对第一财经分析称，需要以公立医院零差率价格为基准，通过集中带量采购形成中选价格，作为医保支付标准参照。同时，强化常规挂网情形下的价格联动约束机制，企业申报的药品挂网价原则上应与本地区主流医保定点零售药店和对应药店互联网“即时达”价格水平保持相当，打通渠道价格传导。另外，在常态化地开展线上线下比价监测时，应更充分地利用网上药店信息透明、竞争充分的特性，促进价格收敛。\u003c!--MID_AD_2--\u003e\u003c!--EOP_2--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_2--\u003e \u003cp\u003e周围举例说，可以通过医保定点协议，督促定点药店主动关注公立医院的价格和其他实体药店和网上药店的价格，以解决同一药品医保价常高于自费价或线上价的“阴阳药价”现象；又如，应关注药企为维护挂网价可能刻意抬高零售端价格的行为，关注因医保结算回款周期长、手续费高，零售药店的终端加价行为，关注线上渠道销售假药、回流药等安全风险问题，杜绝以安全性为代价而形成的价格优势。\u003c!--MID_AD_3--\u003e\u003c!--EOP_3--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_3--\u003e \u003cp\u003e市场监管总局价格监督检查和反不正当竞争局局长姚雷在前述发布会上表示，目前，该局已构建了“全链条价格监管体系”，覆盖原料药流通、成品药生产销售、医疗机构和药店购销等各个环节。\u003c/p\u003e
